COPD cell therapy offers value for patients with moderate-to-severe disease (GOLD grade II–III) with significant breathlessness limiting independence and quality of life, and who have optimised conventional therapy (inhaled bronchodilators/corticosteroids, smoking cessation, rehabilitation). Cost (€5,500–7,000 per infusion) is reasonable compared to lifetime COPD management costs (€2,000–5,000/year in advanced disease) and disability support. Response probability (~50% meaningful FEV1 improvement, ~60% dyspnoea relief) justifies consideration for motivated patients. Limitations: improvements are modest (5–15% FEV1 gain, not normalisation); very advanced disease (FEV1 <25%) responds poorly; and long-term durability is uncertain. Best-fit candidates are age 50–75 with GOLD grade II–III COPD, optimised conventional management, motivated for rehabilitation, and accepting modest, gradual improvement in breathing and exercise tolerance over months rather than cure or dramatic reversal of emphysema.
Chronic obstructive pulmonary disease (COPD) involves irreversible loss of lung parenchyma (emphysema) and small-airway inflammation, progressively restricting airflow and oxygen exchange. Conventional therapy (bronchodilators, corticosteroids) suppresses inflammation but does not repair alveolar damage. Stem cell strategies target structural restoration: placental mesenchymal stem cells (MSCs) secrete hepatocyte growth factor (HGF) and vascular endothelial growth factor (VEGF) that stimulate alveolar regeneration and angiogenesis, partially reconstituting gas-exchange surface. Exosomes from MSCs reduce alveolar inflammation and oxidative stress. The therapeutic hypothesis challenges the dogma of irreversibility; evidence suggests modest alveolar regeneration is possible, particularly in early-to-moderate disease. Thirty-four registered trials investigate COPD cell therapy; ten actively recruit, indicating expanding clinical interest.
