Embryonic Stem Cells: how this cell therapy works, evidence & cost

Embryonic stem cells (ESCs) are pluripotent cells isolated from the inner cell mass of the blastocyst, capable of self-renewal and differentiation into all three primary germ layers—ectoderm, mesoderm, and endoderm—and theoretically any mature cell type in the body. Their pluripotency is their defining strength and their primary liability: in vivo, undifferentiated ESCs carry risk of forming teratomas (disorganised tumours containing multiple tissue types). Human ESCs became available in 1998, catalysing decades of regenerative medicine research, but their use raises ethical questions related to embryo derivation that vary by jurisdiction. Directed differentiation protocols now allow relatively efficient generation of specific cell types—cardiomyocytes, dopamine neurons, pancreatic insulin-secreting cells—with progressively higher purity and function. ESC-derived products are primarily research tools and developmental models; clinical translation has been cautious and limited. A landmark trial of ESC-derived retinal pigment epithelial cells for age-related macular degeneration was discontinued after one patient developed an adverse event, illustrating the challenges of moving pluripotent cells into humans. Current therapeutic interest focuses on terminally differentiated, lineage-restricted progeny derived from ESCs rather than the cells themselves. The advent of induced pluripotent stem cells (iPS cells)—reprogrammed adult cells with ESC-like properties—has partially displaced ESC research for many applications, though ESCs remain valuable for fundamental developmental biology and quality benchmarking.
