The 5 stages of Parkinson’s disease: a clear guide (2026)

A plain-English guide to how Parkinson’s disease progresses, based on the Hoehn & Yahr scale. Understand each stage, what changes, and where treatment fits.

In short: Parkinson’s disease is staged clinically using the Hoehn & Yahr scale, which describes how motor symptoms spread and how balance and independence change over time. Progression varies widely between people and modern treatment can slow functional decline.

The 5 stages of Parkinson’s disease

StageWhat happens
Stage 1 — One-sided symptomsMild symptoms on one side of the body (tremor, slight rigidity or slowness). Daily life is largely unaffected and the condition is often barely noticeable.
Stage 2 — Both sides affectedSymptoms appear on both sides; posture, walking and facial expression change. Tasks take longer, but the person remains independent.
Stage 3 — Balance affectedMild-to-moderate disease with loss of balance and slowed reflexes; falls become possible. Still physically independent, though daily activities are harder.
Stage 4 — Severe disabilitySymptoms are severe and limiting. The person can still stand and may walk unaided but needs help with many daily activities and usually cannot live alone.
Stage 5 — AdvancedStiffness and balance problems confine the person to a wheelchair or bed unless assisted; round-the-clock care is required.
