Hans Clevers' discovery of the Lgr5 marker in intestinal crypts and his subsequent development of organoid culture systems transformed understanding of adult stem cell biology and tissue regeneration. His identification of Lgr5+ cells as the active intestinal stem cell population established a molecular handle on a tissue-specific stem cell, enabling prospective isolation and study. Clevers developed three-dimensional organoid culture from single Lgr5+ cells, generating self-organising structures that mimic tissue architecture and undergo self-renewal—a paradigm now widely applied across tissues including stomach, pancreas, liver, and kidney. Organoid technology enables rapid prototyping of cell therapies, disease modelling, and personalised medicine approaches; patient-derived organoids can be expanded, genetically corrected, and reintroduced. Clevers' work validated that adult stem cells in their native tissue context undergo autonomous self-renewal and differentiation according to developmental rules that can be recapitulated in simplified culture systems. This insight shifted focus from embryonic pluripotency toward tissue-resident adult stem cells as intrinsically powerful regenerative agents. His discoveries have practical implications for regenerative medicine—intestinal organoids are now entering clinical trials for short-bowel syndrome and inflammatory bowel disease—and for fundamental biology, where organoids serve as living models of tissue development and disease. Clevers' legacy encompasses both molecular discovery and methodological innovation that continues to shape regenerative medicine strategy.
