Hair regrowth or density stabilisation (cessation of hair shedding) is reported in 50–65% of treated cohorts over 6 months. Complete reversal of androgenetic alopecia to pre-loss density is rare; most benefit manifests as slowing loss and partial thickening. Younger patients (age <45) and those with earlier-stage hair loss show higher response rates. Genetic predisposition and DHT sensitivity remain dominant factors; exosomes modulate but do not eliminate androgenic drive.
Representative trials document hair density and thickness metrics using phototrichography and image analysis. Phase I–II cohorts report mean hair density increase of 15–35% over baseline in 50–65% of participants within 4–6 months of intradermal exosome injection or topical serum application. Terminal hair transition (vellus to terminal fibre diameter) occurs in 30–45%. Blinded assessor ratings and patient self-assessment show concordance in approximately 60–70% of cases. Durability studies extend to 12–18 months; some re-growth stabilises, whilst other patients require re-treatment.
