Neurogenic cells represent differentiated or lineage-restricted progenitors derived from neural stem cells or reprogrammed from non-neural sources, engineered to progress toward functional neuronal and glial phenotypes. These cells are typically generated in culture through directed differentiation protocols using specific growth factor combinations—often including sonic hedgehog, fibroblast growth factor, and bone morphogenetic protein antagonists. Their defining feature is commitment to a neural trajectory, though they typically retain some plasticity prior to terminal maturation. Stem-cell-derived neurogenic cells have been applied experimentally in spinal cord injury, where they may promote remyelination or provide trophic support, and in neurodegenerative disease models. Direct replacement of lost neurons remains technically challenging because neurons integrate into complex circuit architectures; current strategies emphasise paracrine support, reduction of inflammation, and restoration of protective glial function. Allogeneic neurogenic cell products have progressed into early-phase human trials, with preliminary safety data emerging, though efficacy endpoints remain uncertain. The blood-brain barrier presents a significant delivery obstacle, requiring either direct intrathecal injection or engineering strategies to enhance cellular transit. Clinical protocols in Parkinson's disease, motor neurone disease, and spinal cord trauma are under investigation, with results expected over the coming years as longer-term follow-up data accumulate.
