Duration of immune reconstitution from stem-cell infusion typically extends 12–36 months, with some patients maintaining stable immune recovery beyond 3 years. T-cell expansion and thymic regeneration occur gradually over weeks to months; best durability is observed when immunological niche damage is limited. Some patients eventually plateau or show gradual immune decline, potentially requiring booster infusions. Continued prophylactic antimicrobial therapy is often maintained during immune reconstitution monitoring to prevent opportunistic infection.
