Exosomes are cell-free signalling vesicles, typically 30–150 nanometres in diameter, released by virtually all cell types and abundant in body fluids including blood, urine, cerebrospinal fluid, and saliva. These membrane-bound particles carry proteins, lipids, and nucleic acids (DNA and RNA) and function as intercellular communication vehicles, transferring molecular cargo between cells over considerable distances. Stem cells secrete exosomes that modulate inflammation, promote angiogenesis, and trigger tissue repair responses—mechanisms that may explain some of the therapeutic benefit attributed to stem cell therapies themselves. Exosome-based therapeutics appeal to developers because they bypass cellular viability concerns, require minimal storage infrastructure compared to live cells, and may cross biological barriers more effectively. The cell-free nature offers regulatory advantages; exosomes sidestep complications of cell immortality or oncogenic transformation. Standardisation remains challenging: exosome composition depends on source cell type, culture conditions, and isolation methodology; batch-to-batch variability is common. Clinical trials of stem-cell-derived exosomes are underway in cardiovascular disease, neurodegeneration, and fibrotic conditions, with early safety data emerging. Efficacy endpoints are less developed than for cellular products, partly because exosomal mechanisms—predominantly paracrine signalling—are harder to measure directly in vivo. Manufacturing at scale while maintaining potency and sterility presents technical hurdles. Current clinical applications remain investigational, and claims of exosomal benefit beyond research contexts should be viewed cautiously unless supported by completed Phase II/III trial data.
